2008-2009 Attendance Verification Information

To receive your financial aid, you are required to have each of your instructors sign an “Attendance Verification Form” verifying that you are in attendance.  Please complete the appropriate form and follow the submission dates listed below.

• Fall 08 Attendance Verification Form
• Spring 09 Attendance Verification Form
• Summer 09 Attendance Verification Form

If you are enrolled in a distance learning course, have your instructor e-mail confirmation of your participation to you and then attach the confirmation to your verification form.

Allow 2 to 3 weeks for your check to be mailed once the attendance form has been submitted.

Verifications received after the initial submission date, will be processed on a weekly basis.

Completed forms must be returned between September 4 and September 10 in order for your check to be available for mailing during the week of September 22 (for the 08/FA and the 08/FA1 terms).

Completed forms must be returned between February 5 and February 10 in order for your check to be available for mailing during the week of February 23 (for the 09/SP and 09/SP1 terms).

Completed forms must be returned between June 9 and June 16 in order for your check to be available for mailing during the week of June 29 (for the 09/SU0 and 09/SU terms).

You must complete a new attendance form each time you receive a financial aid disbursement (this includes any late start classes you may be enrolled in).

Obtain your signatures AFTER completion of the second week of each class.

Attention Student Loan Recipients

The above dates apply to students whose loan disbursement date is scheduled for September 5 for the fall semester and February 6 for the spring semester.  Students who have a later loan disbursement date, will need to have their attendance verification completed on or after the scheduled loan disbursement date.
